Title: Medicine/Surgery-LPN/LVN/RPN

Location: Chilliwack, British Columbia – The sports-fishing center of the Fraser River, Chilliwack is also known for its extreme sports, particularly mountain biking on trails with names like The Dilemma.

Description: Under the direction of the General Manager, the LPN is the role model and team leader who coordinates the activities and duties of those who deliver Home Support Services.
The LPN will assess and liaise with clients regarding their home support needs & ensures that the Home Support Service delivered reflects the Personal Service Plan of the client.

Responsibilities:
1. Communicate with client on admission, and regularly thereafter, to formulate a Personal Service Plan (PSP).
2. Communicate PSP with Home Support Workers (Care Aides).
3. Administers prescribed medications and treatments within the limitations of the College of Licensed Practical Nurses of BC. (CLPNBC).
4. Ensures the Home Support service given respects the client's right to self direct.
5. Discuss with the client any noted change to personal care needs, discuss with client the need to seek medical attention as needed, amend PSP if applicable. Discuss with family at client request or if PSP indicates.
6. Actively participates in facility programs, meetings, committees and educational in-services.
7. Maintains documents according to established standards, regulations, policies and procedures, ensuring confidentiality is maintained according to Facility policy.
8. Liaises with other disciplines and departments, as indicated by the PSP.
9. Supervises and assists other care staff. Participates in orientation and ongoing education as required, with the exception of any Transfer of Function which will be performed by the RN.
10. Performs duties indicated in the PSP, which include but are not limited to, all Activities of Daily Living.
11. Ensures equipment and supplies are stored and maintained according to policy and procedure and reports unsafe or faulty equipment to appropriate manager.
12. Maintains an up to date knowledge of facility policies and procedures, particularly those concerning the administration and recording of medications, safety and emergency procedures.
13. Encourages and adheres to a holistic philosophy of wellness, recognizing that each client is Self Directing & is capable of making choices about his/her life these choices will be respected.
14. Will develop and Exit Plan, as per facility policy, when the client is no longer able to Self Direct.
15. Utilizes the Registered Nurse as a resource for complex situations as per the policy of the Facility.
16. Performs other related duties as required.

Qualifications:
1. Graduate of an approved program for Practical Nurses and holds a current full scope License with the College of Licensed Practical Nurses of BC.
2. Valid First Aid Certificate.
3. Two years recent experience as a practicing LPN, in a long term care setting that included the distribution of medications.
4. Ability to communicate effectively verbally and in writing in the English language.
5. Good interpersonal skills and ability to effectively interact with the various individuals involved in providing holistic care in a flexible client focused environment.
6. Ability to operate related equipment.
7. Must be in good health and free from communicable diseases, physically able to perform repetitive tasks, lift, squat, bend, stand and walking as required to perform the duties of the position.
